Need a landing page design.

This is what Live Well Marketing LLC was looking for in their landing page design

We are currently trying to develop a landing page for our www.dynamiclifenutrition.com website.

Objective: To peak a level of interest in Medical Practices to sign up to our program and promote our line of vitamins and supplements. We will be concentrating on email marketing as well as fax or direct mail to drive medical facilities to our landing page.

Page requirements:
1. A slider on the top portion showing our line of products from the dynamiclifenutrition.com website.
2. An "about us" area to explain who we are and what we offer.
3. An area for a grid so that we can show potential earnings for the practice as part of our program.
4. A Testimonial area for a couple of testimonials
5. A scroll area where they can see our contract
