Postcard Design Brief

We are a general & cosmetic dentistry practice in Jacksonville, FL. http://www.mandarindentalpros.com/

We are trying to improve our online presence with Google+ reviews. We have great reviews other places, but want to encourage our patients to leave one on our G+ page using a QR code and some short links to our G+ page and Customer Lobby (the other platform we use for reviews).

Requirements

Must have

• We are not mailing this postcard- do not leave space for mailing address or stamps. We want our card oriented vertically. We must have the design printed twice (both sides) on a standard postcard. We need our logo at the top followed by, "Thank you for your business. We highly value your feedback. Review us on Google+" (Use the G+ button instead of "Google+")
  
  Next we need the QR code we have provided. Under it "Scan this code to review us on Google+" (No G+ button this time).
  
  Then, "Don't have a Google account? Review us on Customer Lobby" with the Customer Lobby graphic attached. Use the URL:http://tiny.cc/SmyleDoctorReview
  
  We would also like 2 or 3 blank lines on the card so that something may be hand-written
  
  The other side should have a larger version of the logo. Underneath it, "We're Social! Find us on:" (use social buttons for Facebook, Google+, Yelp)
