Word Template Design Brief

My company name is Heartland Insurance Services. We are a full service insurance brokerage.

I need an ACH Form (which is attached) to be created with my logo (which is also attached). Please make the ACH Form fillable for info to be entered manually.

In addition I need the Example Quote created with the same color scheme as the Heartland Logo. Please create the Example quote as a word document so I can edit the quote and save as a PDF. You can also send me different quote ideas as long as they have the same information on the quote I have attached.

For the Example Supplemental you can leave everything as is and just change the Brookhurst Logo to the attached Heartland Insurance Services Logo.
