Web Design Brief

I have a landing page script which has no design element whatsoever.
I need someone creative to take the script and make a design, including providing the HTML script once finished.

The landing page will include a video sales letter, some testimonials, 2 buttons (one which will show a pop-up with 3 confirmation checkboxes and another button which when pressed will open an embedded calendly window or pop-up.

Most importantly, the landing page must be designed with best conversion practises in mind.

The attached script is a draft, but the final copy it will be very similar in terms of length and content.
