Logo Design Brief

I am an Optometrist and I need a logo that I can put on my website and business cards. I don't sell glasses or contact lenses. I concentrate on eye exams and taking care of my patients. My motto is "Clear vision begins with healthy eyes". I want something modern and elegant.
My business name is Dr. N. Ahmad & Associates. To be honest I'm not sure how I want that incorporated in the logo.
