Graphic Design Brief

I am looking to create a chart/graph to be used to chart a patient's progress before and after using our 15-day detox program. We will be looking for 3 different graphs to be made.

1. A chart showing the before and after symptom scores of 12 patients in a small clinical trial we did. We will use this one to show prospective patients.

2. A chart showing one single patient's "total" symptom scores before and after the detox. We will use this with each patient, before and after they do the detox.

3. A chart showing one patient's before and symptom scores in more detail, showing their scores in each one of the symptom categories. We will do this with each patient, before and after the detox.

For #2 & #3 above, we would want a master template that we could add the scoring for each new patient ourselves.

See attached for rough samples.

Each of these 3 charts should be the same style, but for now, just do chart #3. And after we decide on the designer, then we will have them do the other 2 charts.

And I prefer a line graph over a bar graph for these. I did not mention that at first. Sorry!!
