Flyer Design Brief

We are looking for a redesign of an existing form - one that fits our new marketing material better and, frankly, a form that looks a lot nicer than the boring form we use today. 8.5"x11". CMYK. Minimum 300 dpi necessary for printing. I have attached our current quote form as well as some other designs that show the direction we are going in. Please note, that this has to be able to have our people manually write in pen on it when we provide it to potential customers. Thank you!
