Ultimately I am looking for a mixture of icons and text, covering the fields included in the spreadsheet.

Can't find a head or tail......for your project..Please post design with a similar example or Can u post a submitted design, I will get you the best . we need a thread to hold...an example or a previous design......Just think why your project has no entries ? I will make it clear...because your brief is not clear also you have not provided any examples....
