Prefer something vector, if possible. CMYK color, Adobe InDesign, Illustrator, or EPS format.

I just sent my design. The text effects, the background and the colours are totally customizable: you can put any font and size; you can change the background texture and colours. If you want to change quickly and easily the template, I recommended using a PSD file, because it is not possible change a vectorized text. With a PSD (Photoshop) format, you can change anything without a lot of effort with the Photoshop Styles.
