Flyer Design Brief

We need a brochure design for our promotional/marketing materials for our medical practice. The practice's name is "Premier Spine and Sports Medicine".
We are based in Hagerstown, MD. Services we currently offer include: Sports Medicine, Pain Management, Acupuncture, and Regenerative Medicine (includes Platelet Rich Plasma and Stem Cell Injections).

The brochure needs to be able to be reprinted as needed.
The text sections of the brochure will need to be able to be edited as the practice grows and we add other services.

I would like to see the following sections:

1) About Dr. Clark:
Dr. Christopher Clark received his medical degree from Brody School of Medicine at East Carolina University. He completed his residency training in Physical Medicine and Rehabilitation at Sinai Hospital in Baltimore, MD. He completed his fellowship in interventional pain management and musculoskeletal medicine at Performance Spine and Sports Medicine just outside of Princeton, NJ. After completing his fellowship, he was an attending physician at Performance Spine and Sports Medicine until fall of 2017. He opened Premier Spine and Sports Medicine in Hagerstown, MD in October 2017. He has a wife and 3 kids.

2) Our Practice Philosophy:
Invidividualized, compassionate, collaborative, and state of the art medical care in Spine and Sports Medicine. At Premier, we do not take a cookie-cutter approach to a patient's complaint. We are committed to providing the best quality care over quantity. We are committed to finding and treating the root cause of a patient's pain. We are committed to avoiding or weaning the use of narcotic pain medications as much as possible.

3) Conditions Treated
Headaches, Neck Pain, Back Pain, Degenerative Disc Disease, Facet Joint Arthropathy, "Pinched Nerves, Sciatica, or Cervical/Lumbar Radiculopathies; Sacroiliitis, Shoulder Pain, Rotator Cuff Tendon issues, Golfer's/Tennis Elbow, Wrist Pain, Carpal Tunnel Syndrome, Hip Pain and Arthritis, Knee Pain and Arthritis, Ankle Sprains and other disorders, Achillies Tendinitis, and Plantar Fasciitis.

4) Services Offered:
Sports Medicine, Pain Management, Acupuncture, and Regenerative Medicine (includes Platelet Rich Plasma and Stem Cell Injections), Botox Injections (for Migraines) -- this section may need to have the most room to allow for brief descriptions for all the services

We would like to see the brochure use the same colors scheme/ideas of the attached logo/

• It must be able to be edited after completion to account for when the practice grows and adds other features.

Nice to have

• Nice to have pictures health and wellness background "vibe" to it.Maybe the "backdrop" has someone running, doing yoga, playing a sport, etc.
• PLEASE SUBMIT BI-FOLD OR TRI-FOLD DESIGNS. FLYERS ARE OK AND CONSIDERED A BONUS BUT BROCHURES ARE PREFERRED.
• ALSO SEE SOME ADDED PICTURES TO CONSIDER FOR INCORPORATING
