We need an infographic that will artistically illustrate our process with providing services and value to our clients. So we can fax blast this information to our clients in order to generate more client leads, by having them call us. We are based in Los Angeles CA servicing clients all over the united states. We provide home health therapy (Mobile Physical Therapists, Occupational Therapists, Speech Therapists, Dietitians, and Medical Social Workers) to home health agencies (Our Clients) Our clients provide provide mobile, nursing, therapy, and medical equipment to their patients in their own home. We would like to see designs geared to explain the value we provided to our clients in an infographic format that can be easily faxed to our potential clients so they call us to set up a contract and utilize our services. Here are the steps to our process. 1. Therapists Is Goes Through a Criminal Background Check. 2. Therapists are trained and shadow another therapists. They are trained how to perform SOC paperwork, How to pass survey, What the Medicare Payment Schedule is, and the the reporting parameters. 3. We notify our clients weekly of our therapists availability. 4. Our Client (Home Health Agencies) Call, Email, Fax, Text, or Message us Requests (THe Address of the patient) We are available Monday through Sunday 8 AM to 7 PM. 5. We then verify the availability of the clinician (PT, OT, ST, LCSW, RD) 6. We accept the referral 7. We Triage the patient (By Confirming the SOC was done, COnfirming the patient's Address, and Obtaining a secondary phone number from the patient. 8. We assign the therapist and informo the client and the new patient which therapists will be providing therapy. 9. The therapists schedules the evaluation (Next Day) 10 Performs the evaluation (within 24-48 hours. 11. Reports their findings via an initial case conference immediately after the evaluation. 12. Continues to treat and document what was treated in accordance to Medicare Guidelines. (Documentation is posted within 12 hours of seeing the patient) 13. Therapists Monitors the patient and Reports Status (By taking vitals, Reporting falls, Reporting ER Visits, Reporting Changes to Medications, Reporting MD orders, and asking the patient if there is another home health agency on the case, or if they have had a change to insurance.) 14. Therapists reassess (Every 30 days, If the patient falls, Goes to the ER, If they need more visits, or if there are any changes to status) 15. Therapist performs DC planning and asks for feedback. (Our office asks if we will be the last discipline on the case and determines if we need to do a discharge (DC) Oasis and a NOMNC) 16. Our team QA's the chart and verify's the therapists credentials (Those are available online 24 hours a day 7 days a week) 17. We invoice the client.
