Logo Design Brief

We need a logo design for an allied health clinic thats called Form Fit and Function Health. We are after a logo that looks like a puzzle. Please see attachment. We want 3 of the following colours (navy, lime, dark grey or sandy). We are specialising in sport injuries. We will also have pilates, chiropractic, physiotherapy and massage. We want the design to fit the name of the company but also look slightly like a spine/health clinic.
