Business Card Design Brief

I need a business card and logo that is "unique", abstract, modern, professional, and trendy. On the back of the card, I would like to have an appointment reminder which includes: the patient's name, the date, time, and a box to check the day of the week of the appointment. Also, on the card, I would like it to say "The eye is the lamp of the body. If your eyes are healthy, your whole body will be full of light."
