Logo Design Brief

I would like to have a dedicated logo for a H&S record
(10 years without accident).
This logo should include our safety logo and the following text below.
The logo will be used for internal communication as well as for external communication.
It should be readable in a 5 cm side square or a 5 cm diameter circle but could be printed on a larger scale.
I need to submit a project by end of next week (April 24). I will need an illustrator file although a jpg is good for the selection process.
