Testing form re-design

This is what a business in United States was looking for in their graphic design

I have 2 new testing forms in a very basic format, I need them updated to look similar to our other testing forms. Attached are the two new forms (Dermatology and GI) as well as a couple of our Original normal testing forms to compare them to. Needs to be in an editable PSD file incase I need to change the verbiage in the future. Also a few addendums to the Dermatology form - Need to add a form field for "Clinical Impression" as well as "Clinical History" with spaces 2 inches to handwrite next to them. Read more
