TVC Values Puzzle Pieces

This is what Trident Vein Center was looking for in their graphic design

I want to use my logo (symbol only, not the words Trident Vein Center) as the basis of the puzzle and incorporate our four values into the logo as four puzzle pieces. I want each puzzle piece to have one value on it and a line extending from the piece. At this point, I would like the bullet points of each value to be listed.

Here are the four values with the bullet points:
